Referrals can be made by contacting us directly via email at placements@nextphasehousing.co.uk or by calling 0121 812 6424. We’ll arrange an initial assessment to determine suitability and availability.

Support is planned through an initial assessment and delivered through an individual support plan. Progress is reviewed regularly with the young person, support workers, and relevant professionals to ensure it remains appropriate and goal-focused.

Support is structured around clear routines, agreed expectations, and regular one-to-one sessions. We work closely with local authorities and relevant professionals through updates and review meetings, adjusting support as needed to ensure each placement remains safe, appropriate, and effective.

Once a referral is received, we carry out an initial assessment to understand needs, risks, and suitability. We then confirm availability and next steps with the referring professional before placement is agreed.

We maintain 24-hour staff presence, conduct regular risk assessments, have comprehensive safeguarding policies, CCTV in communal areas, and work closely with local police and emergency services.

House rules focus on safety, respect, and shared living. They include expectations around behaviour, visitors, noise levels, cleanliness of shared areas, safeguarding, and compliance with health and safety requirements. All rules are explained during induction and applied fairly to maintain a stable and supportive environment.
